Questions for your doctor or nurse
Call (985) 446-2680, option 8 to leave a message for your doctor or nurse. Please include patient name, date of birth and a contact phone number in your message. Most messages will be answered within 24 hours, but please allow up to two business days to receive a response. If your message is urgent and your physician is not in the clinic on the day you call, the nurse's voice-mail greeting will ask that you dial 0 to be connected to a receptionist. Otherwise, non-urgent messages will be answered when your physician returns to the clinic.
For questions about your statement or to make a payment, please call (985) 493-2847 and our billing staff will assist you.
For questions about your insurance coverage or insurance claim, please call (985) 446-2680 extension 1001 and our insurance staff will assist you.
Medical records request
Please print and complete the Authorization to Release Records FROM Family Doctor Clinic here and mail or fax the signed form to Family Doctor Clinic. Please give a detailed purpose for your request (e.g. legal, insurance, personal, etc.) as this may determine if copy charges will apply. Our third-party copy service fills requests once a week. To check the status of a previously-submitted request, please call (985) 446-2680, option 5.
